By registering with the Greater Worcester Worcester Adult Soccer Association ("GWASA") I confirm that I have read and agree to the release and disclaimers below, and I recognize that I am giving up substantial rights by agreeing/signing.

In consideration of being allowed to participate in any way in the GWASA and/or the Massachusetts Adult State Soccer Association ("MASSA") and/or U.S. Adult Soccer Association (“USASA”) athletic/sports programs and related events and activities, the undersigned:

Agrees that prior to participation, he/she will inspect the facilities and equipment to be used and, if he or she believes anything is unsafe, he/she will immediately advise their coach and match official of such conditions and refuse to participate.

Understands that there is NO supplemental insurance coverage provided through the registration fee and agrees to assume all responsibility for hazards incurred in the conduct of activities as well as transportation to and from activities.

Acknowledges and fully understands that the participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to the GWASA or not reasonably foreseeable at this time.

Assumes all the foregoing risks and accepts personal responsibility for the damages following such injury, permanent disability, or death, or the injuries which I may cause others.

Agrees to abide by all the rules, regulations, policies and decisions of the U.S. Soccer Federation (“USSF”), the USASA, the MASSA and the GWASA to which the undersigned is registering and agrees to accept any disciplinary action taken by the USSF, the USASA, the MASSA and the GWASA to which the undersigned is registering.

Releases, waives, discharges, and covenants not to sue the USSF, the USASA, the MASSA, and GWASA, its affiliated clubs, their respective municipalities, administrators, directors, officers, agents, coaches, and other employees of the organization, other participants, volunteers, referees, sponsoring agencies, sponsors, advertisers and, if applicable, owners and leasers of premises used to conduct the event, all of which may be referred to as "RELEASEES", from demands, losses or damages on account of injury, including death or damage to property, that is caused or alleged to be caused, in whole or in part, by the negligence of the releases or otherwise, to the maximum extent permitted by law.

Acknowledges and fully understands that, pursuant to various by-laws with the USSF, the USASA, the MASSA and GWASA, that if a participant decides to file a lawsuit against any of the above named entities, that they will be suspended from all soccer related activities under the umbrella of the USSF until such litagation is completed and possibly longer.
